Cultural Recreation Advisory Board Application - High School Student Member

  1. CULTURAL RECREATION ADVISORY BOARD APPLICATION

    Fill out the entire application and submit the required resume in a PDF format. The Advisory Board appointment term is for two years.    We are recruiting for 2 or more Student Members with a 2-year term.  Graduating Seniors terms will end upon graduation.

    You must be a resident of the City of Westlake Village to apply.  If you are appointed, you will be required to file a Fair Political Practices Commission (FPPC) Conflict of Interest Statement within 30 days of appointment, annually thereafter and within 30 days from leaving office.

    Additionally, all individuals selected for appointment will be required, within 14 days of appointment, to complete a LiveScan/Background check as mandated by State Law.  The City will pay for the LiveScan costs. Applicant must receive a clear background check BEFORE participating in their first meeting or event as a Cultural Recreation Advisory Board Member.  If an applicant does not pass the background check, they will not be appointed to this committee.

  3. For all High School students applying, please indicate the High School you are attending and your current grade level (Sophomore,  etc.)

  4. Please include a brief summary of your personal qualifications, including education, past or current employment, general community/civic involvement, etc., which may be considered pertinent in serving on this committee.
  5. Please include a brief summary of your fields of interest pertaining to potential appointment to the Board. This may include specific events, special projects, or sub-committee interests.

  6. If appointed, you will be required to attend the Cultural Recreation Advisory Board Meetings, which are held in-person.  The group normally meets at least 6 times a year, the first Thursday of the Month at 6:00 pm - City Hall/Community Meeting Room.    The meetings are in-person meetings.

  7. Members of the Board must file a Statement of Economic Interest, Form 700, as the position is listed in the City’s Conflict of Interest Resolution. Assuming Office filing due within 30 days of appointment, annual thereafter and when term ends, a leaving office statement within 30 days.

  8. All individuals selected for appointment will be required to complete a LiveScan/ Background check for the City as required by State Law.  This must be done within 14 days of City Council approving your appointment.  You may not participate in any meetings or volunteer to work an event until your background check is completed.  The City will pay for the LiveScan.

    If an applicant does not complete or pass the LiveScan, their appointment will be rescinded.

    If you are selected for appointment, you will need to file a Conflict-of-Interest Statement as this position is listed in the City's Conflict of Interest Code Resolution.  Additionally, you know that you will be required to complete a LiveScan/Background check, as mandated by State Law, and success pass the background check for your appointment to be official.  

    Failure  to complete mandated items will result in removal from the Cultural Recreation Advisory Board.
